Job Overview:
The Patient Care Coordinator plays a key role in delivering a positive and seamless patient experience. This position supports front desk operations, patient flow, appointment scheduling, and coordination across the care team to ensure efficient and timely delivery of services.
The ideal candidate is organized, patient-focused, and able to effectively manage multiple priorities while providing professional and compassionate support to patients and healthcare staff.
Key Responsibilities:
As a Patient Care Coordinator, you’ll play an important role in creating a smooth, welcoming, and efficient patient experience. You’ll serve as a key point of contact for patients while coordinating schedules, supporting patient flow, and partnering with clinical and optical teams throughout the patient journey.
Welcome and assist patients, customers, and vendors with a friendly and professional approach.
Serve as a primary point of contact for patient questions, requests, and general inquiries.
Manage incoming calls, emails, and digital communications promptly and professionally.
Schedule, confirm, and reschedule patient appointments while helping optimize provider schedules and fill same-day openings.
Support appointment reminder and confirmation processes to help reduce no-shows.
Verify patient demographics and insurance eligibility and maintain accurate information in the EMR.
Assist patients with questions regarding insurance coverage, benefits, and financial responsibilities.
Prepare patient charts and required documentation to support an efficient visit.
Process co-pays, payments, charges, and account information accurately in the EMR.
Monitor patient flow and provider schedules, proactively identifying and addressing delays, gaps, or scheduling conflicts.
Keep patients informed about wait times, delays, and next steps throughout their visit.
Coordinate with clinical and optical teams to support smooth patient transitions and continuity of care.
Support patient retention through follow-up, recall, and reactivation outreach, including annual and medically necessary appointments.
Help maintain a clean, organized, and patient-ready front office environment.
Perform other duties and responsibilities as assigned by the Practice Manager, Assistant Practice Manager, and/or Optometrist.
